Position Summary

Responsible for the delivery of medical social services to Mercyhealth at Home - Winnebago patients.

Performance Standards

  1. Conducts the initial psychosocial assessment of the patient and family, and establishes a plan of care.

  2. Provides age specific social work therapeutic interventions through counseling and education to both the patient and family/caregiver to maximize their strengths and coping capacity.

  3. Assists in negotiating fiscal systems, particularly government entitlements. Knowledgeable of various resources available to patient and families.

  4. Advocates for patient and family to assist them in receiving the appropriate assistance needed to cope with their identified problems. Promotes self determination and self advocacy whenever possible. Provides short term direct counseling and refers persons with long term counseling needs to community agencies.

  5. Provides continuous evaluation and monitoring of services through documentation and consultation.

  6. Collaborates with disciplines within the agency to ensure a comprehensive and holistic approach to care.

  7. Attends team departmental meeting regularly to provide input during patient case conferences and to educate team members regarding social work interventions that can be provided.

  8. Assists in agency-centered activities such as quality improvement, orientation of new employees, inservice education, skills validation and annual performance review of peers. Promotes Mercyhealth within community, networking with colleagues, agencies and other resources.

  9. Provides bereavement support.

  10. Participates in QI activities. Maintains continuing education CEU's for licensure requirements.


Masters of Social Work (MSW) from an approved graduate school of social work

Work Experience

One year social work experience in a healthcare setting; Experience and ability to facilitate groups


Currently Licensed (LCSW/LSW) in the State of Illinois; Current CPR certification; Requires a valid driver's license (for a minimum of 12 months), a satisfactory driving record (as deemed by Mercyhealth), and proof of automobile insurance equal or greater to that required by the state of Illinois.
