Catholic Charities of the Archdiocese Lead Home Care Aide Supervisor in Chicago, Illinois

Essential Functions of the position are detailed below, and include any physical requirements below that.1. Supervises and works directly with Program Assistants in coordinating client care with referral sources, clients and employees in accordance with the Illinois Department on Aging (IDOA) standards.2. Assists the Director of Home Care with overall management of the Home Care Program and functions as back-up Director in the absence of the Home Care Director.3. Supervises an average of 100 Home Care Aides. Responsibilities include but not limited to matching Home Care Aides with clients, scheduling and modifying Home Care Aides hours and evaluating performance of Home Care Aides4. Completes home visits up to four (4) days each week to monitor both Home Care Aides and senior clients. 5. Conducts well-being checks on seniors when completing home visits and reports any changes in seniors' physical or mental status to their supervisor. If abuse is apparent or perceived Elder Abuse Hotline is contacted. 6. Works closely with Staff Development Coordinator to ensure that Home Care Aides receive all training necessary to perform their job responsibilities in accordance with all agency, licensing, accrediting and funding body requirements.7. Works closely with client, clients families and others to guarantee that client is receiving best available care and adhering to any specific plan in place.8. Works collaboratively with departmental staff and others to complete in accordance with agency, licensing, accrediting and funding bodies all required sheets, logs, forms or other required documents.9. Responsible for and/or makes the needed recommendations for the discipline and performance evaluation of staff; responsible for assigning work to staff and ensuring the work is performed in an efficient, prompt, professional and accurate manner and provides guidance, when necessary.10. Handles all client and employee complaints as well as crisis situations in consultation with his/her supervisory chain and in compliance with Agency/Programmatic policies and procedures and treats each case with sensitivity and expedience.

  1. Work in conjunction with the billing department to ensure authorizations are up to date, accurate and billable.

Education and Experience Requirements:

Relevant Education: Preferred: Master's Degree Geriatrics, Social Work or Health related fieldMinimum: Master's Degree

Relevant Experience:Preferred: 3 years experience in geriatric home care including supervisory experience.Minimum: 2 years experience in geriatric home care including supervisory experience.
